A.1 By booking an appointment with UK Cannabis Clinic(UKCC), you agree to your data being held and processed by UK Cannabis Clinic, their prescribing partners, their medical partners and research partners.
A.1.i All data held by UK Cannabis Clinic and their clinical partners is held and processed at your specific request in order to receive medical cannabis products under our protocols.
A.1.ii All data on your clinical pathway, all notes from clinicians and data presented to us by yourself will be shared anonymously with the lead clinicians or consultants from each arm of research. This will enable the data to be anonymously analysed.
A.1.iii For the avoidance of doubt, all data given freely to the UK Cannabis Clinic may be used for primary research purposes in an anonymous way.
A.1.iv Patient identifiable data will not leave the confines of the UK Cannabis Clinic system, or their pharmacies/clinics/dispensaries.
A.2 The UK Cannabis Clinic is private medical service, not affiliated with the NHS.
A.2.i Despite not being affiliated with the NHS, UKCC has a duty of care to inform your NHS General Practitioner of any/all treatments we are providing/prescribing.
A.2.ii As part of the consultation, a letter will be generated which we ask you hand into your general practitioner. This is your personal responsibility.
A.3. What is included in the consultation:
A.3.i The consultation gathers advanced data about you and your health. This data is used to make decisions on what sort of products would be most suitable for you.
A.3.ii You are required to be honest and complete with your answers to our questions, and it is your responsibility to tell us a complete medical history where it is required.
A.3.iii The consultation will be with a registered clinical professional, who will have access to our Prescribing Services Directive. This is overseen by a number of expert doctors, consultants and prescribers in the medical cannabis field.
A.3.iv As part of the consultation process, your information will be transferred to a prescriber, who will review and may or may not issue a prescription to your UKCC.
A.3.v Unless specifically requested, UKCC are not responsible for the dispensing of the prescription, however, it must be dispensed by a UKCC accredited pharmacy or clinic.
A.3.vi For the avoidance of doubt, consultation fees paid to UKCC do not cover the cost of the medication. A prescription for 3 months in included in the first consultation, if it is appropriate to be issued.
B.1 Your right to cancel.
B.1.i. If an appointment is booked, and you cannot make it – contact the clinic directly. They will do their best to accommodate you. However, fees apply when you cancel and do not rebook. The following scale applies:
Cancelled 7 days or more before – Full refund
6 – 3 days - 50% refund
2 days or less - 0 refund
B.1.ii We will repay via the method you paid only. There are no cash refunds.
B.1.iii Patients requiring refunds must contact their local UKCC and have the credit or debit card available that was used to book.
C.1. Our right to cancel
C.1.i We will make every effort to provide the Services on the date that we have specified. However, we cannot promise this and we reserve the right to refuse your consultation or to cancel or change the date of your consultation. There may be delays or cancellations for any reason, such as because of an event outside our reasonable control, for operational or technical reasons or because your clinician does not think it is in your best interest for medical reasons. If this happens, we will give you as much notice as possible. Where possible, as appropriate, we will always try to rearrange any appointment or consultation dates with you. Where we are required to cancel the appointment or refuse consultation, any advance payment for Services that have not been provided will be refunded to you. We will refund these amounts by electronic transfer only to the cardholder or person who made the original payment.
D.1. Insured patient (patients with private healthcare insurance)
D.1.i You agree to pay for your care.
D.1.ii We will provide a suitable invoice for you to separately make a claim to your insurance company.
D.1.iii Where the UKCC is a veterinary clinic, and claims can be automatically processed this will be completed where possible at the time of consultation or thereafter. This is a local arrangement between you and the UKCC.
E.1 International Patients
E.1.i By signing a Consent Form and agreeing to these Terms you confirm that you have the necessary documentation to enter the UK and that you meet all relevant immigration criteria. You also confirm that you have made adequate arrangements to pay for your care.
E.1.ii International patients must confirm to the clinician that they are currently resident in the United Kingdom.
E.1.iii UKCC are not responsible for any harm, or loss, incurred to you due to taking any of the products prescribed to you across international borders. It is your responsibility to check the legal status of each product in your country before travelling.
F.1. General Disclaimer
F.1.i All products provided by UKCC are investigational, or off-license. These products have no UK marketing authorisation for the conditions they are being used for. Both the patient and the UKCC must clearly understand that unlicensed treatments can make no claims for efficacy and that any harm, howsoever caused, by the products or treatments cannot be attributed to UKCC, it's doctors, clinicians, pharmacists or nurses.
F.1.ii In order to see a clinician in the UKCC the patient formally and completely waives all liability for harm, financial losses (or other) to themselves (or others ) caused directly or indirectly through their association with UK Cannabis Clinic, their partners or protocols.
